WEDDING GOWN/OTHER GOWN DONORS: Please fill out box 2 we need to know who received your gown. List the Name and complete address. Thank you! We will have your receipt to you before or by years end. HAIN Volunteers: 1.Please make sure to add the Fair Market Value (FMV) of your donations so we can use them for our records even if you state you do not want a receipt. You can also include shipping cost on the form. 2.Make sure you list the full name and address of the person/place receiving your donation. If this is several peoples donations please list that on this form somewhere, also make sure you fill out a form per person donating-so we can offer each donor a receipt. 3. After submitting form you will see a confirmation page, be sure to look it over and see if everything is accurate. If not contact me or refill out form. 4.
